CDPH / CDC Request Form for COVID Over-The-Counter Tests

Which organizations are eligible to apply?
  • Public, charter, and private schools, except those located in Los Angeles county
  • Any summer camp serving K-12 students, including those located in Los Angeles county
  • LEAs in Los Angeles county should reach out to the LA Department of Public Health for more information on how to receive tests at
    acdc-education@ph.lacounty.gov
How do I reorder?
  • Please reorder by filling out the form below
  • Organizations must attest to distributing at least half of the tests received before being able to request additional tests.
How do I store tests? 
  • Tests must be stored in a temperature stable environment (must be stored between 35.6-86°F)
What is the timeline for submitting and receiving tests?
  • To be eligible for the current week’s disbursement, please complete the form by 12 PM on Fridays. The last date to order is June 17, 2022 so please order for your summer needs.
  • Applications received after the deadline will be considered for the following week.
  • After the weekly submission request, CDC will inform CDPH of the selected recipients within 5-7 business days. CDPH will inform recipients if they were selected.
  • Tests typically take 10-14 business days to be delivered after submission deadline.
  • The CDC will also be reaching out directly to the point of contact with shipping information
Tests are not guaranteed.
The federal government has advised CDPH that it may not be able to fill all of the requested allocations.

How can I use the tests?
“Over the Counter (OTC)/At-Home tests” can be used at home by individuals or parents/guardians. If they are used outside the home, they must still be completed by individuals or parents/guardians. This can be observed by LEA or camp staff. However, if they are performed by LEA or camp staff, they are subject to federal laboratory regulations (“CLIA”) and require special training. If you plan to use OTC tests in this type of scenario, your organization needs to apply for use for CDPH’s School CLIA waiver if your organization serves K-12 students. These are large orders and cannot be delivered to PO Boxes. Your address must correspond with your school address and will be verified. We are unable to deliver to a residential address. If there might be an issue confirming your delivery address belongs to your school, for example if your school recently relocated, please include that information in the additional delivery instructions text box below.
